Instead of filling in the application by hand, parents are encouraged to fill in the Word version of the registration form on their computer, print it off and send it in with their cheque.
Player Declaration of Residence
- Please email our Registrar, Genny Billingsley ( genny-billingsley@cumberlandminorhockey.ca ) for assistance with completing the additional transfer forms.
House League Coach Application and Police Check
If you would like to Coach a House League team, please follow this procedure:
- Download, print and filled out the coach application form and police check:
- Mail or deliver your completed application to:
- Kelly Lafranchise
- Vice President - Hockey Operations
- 1526 Rumford Drive, Orleans
- or scan and email to:
- Please submit applications by August 31.
- Police Checks:
- please send email to kelly-lafranchise@cumberlandminorhockey.ca for a letter to give to the Ottawa Police to waive the fee.
